Steps to Unlock Dragon's Dogma 2's True Ending
Get Empowered Godsbane Blade in Main Quest: A New Godsway.
During the Main Quest: A New Godsway, you must get the Empowered Godsbane Blade by delivering Wymslife Crystals to Ambrosius at the Forbidden Magick Research Lab. The Empowered Godsbane Blade is a key item you will need to unlock the game's "true" ending.
Progress to Final Main Quest: Legacy
After getting the Empowered Godsbane Blade, continue with the story until you reach the Main Quest: Legacy (final). Agree to battle The Dragon to ride on his back. During the flight section that takes you to the combat area, climb down onto The Dragon's chest and head for the glowing opening.
Use the Empowered Godsbane Blade while clinging to The Dragon.
Open your inventory while holding on to The Dragon near its chest and use the Empowered Godsbane Blade. This will trigger a cutscene that takes you to the Unmoored World.
Head to the glowing spot near The Dragon's head to view the true ending.
Once you regain control of your character again, approach the glowing spot near The Dragon's head. This will then trigger the game's true ending.
